Admissions Checklist

1. HIGH SCHOOL DIPLOMA
The applicant must be a high school graduate or possess the recognized equivalent of a high school diploma. The applicant must provide documentation of graduation from high school in the form of a valid high school transcript, diploma, or other acceptable documentation including a transcript attestation form, which confirms that the applicant meets or exceeds the academic achievement equivalent to a standard high school diploma earned in the USA as defined by the State where the diploma was earned. All documents from foreign countries must be translated into English and evaluated to be equivalent to or higher than a USA high school diploma by a credential evaluation service.
2. IMMUNIZATIONS
The applicant must send prior immunization records including documentation of Hepatitis B injections #1 and #2, a tuberculosis (TB) test within the past 6 months, proof of immunity against measles, mumps, and rubella (MMR), and proof of meningitis vaccination or a signed declination within 30 days of the program start. Serological evidence of immunity may be submitted for Hepatitis B, MMR, varicella (chickenpox), and tetanus, diphtheria, and pertussis (Td/Tdap). A Tdap booster is required once, with Td boosters every 10 years. Initial two-step TB testing is required for those without prior testing, and annual one-step testing is required thereafter. Health clearance form must be submitted upon enrollment along with vaccination records to adom@adomacademy.com.com or complete this immunization form.
3. CITIZEN OR LEGAL RESIDENT REQUIREMENT
The applicant must provide a valid form of picture, name and address identification. A valid and current driver's license, United States passport or Permanent Resident card are acceptable forms of identification.
4. REQUISITE (NON-CORE) COURSES
The following pre-requisite courses must be successfully completed in one of two ways:

1) Earning college level credit with a grade of “C” or better, prior to entering the program: Anatomy and Physiology, Algebra, Statistics or Higher Math, General Physics, Communication Skills (This can be met with English or Speech).​
2) Taking the ADOM requisite courses that are offered 2 weeks prior to program commencement that include Anatomy and Physiology, Mathematics, General Physics and Professional Communication.
5. TRANSCRIPTS
Official high school transcripts or other high school equivalent records must be submitted as part of the application process. All college transcripts (if applicable) are required for applicants applying to transfer credit. Official, sealed transcripts will be required prior to commencement of the program, however, applicants can email preliminary transcripts to adom@adomacademy.com. ​
6. ADMISSIONS TESTING
Admissions Testing: ADOM Ultrasound School requires applicants to take the ADOM Anatomy & Physiology Entrance Exam. The A&P test is designed to assess foundational knowledge in anatomy and pathology relevant to ultrasound practice. This open-book test features questions requiring short answers that demonstrate a basic and practical understanding. If you have completed an A&P course with a grade of B or better from another college, you can opt-out of this test by providing verification. is no fee to take the test. Email adom@adomacademy.com to request the A&P entrance exam.
7. LETTERS OF RECOMMENDATION
Applicants are required to submit two letters of recommendation from teachers, employers, or other individuals (not relatives or co-workers) who can provide insight into their character, academic abilities, or relevant experiences. Send letters of recommendation to adom@adomacademy.com.
8. PERSONAL STATEMENT / ESSAY
ADOM requires applicants to write a personal statement or essay describing their patient care experience. For individuals who do not have formal patient care experience in a clinical setting, they are required to complete an essay detailing their personal patient care experience, such as caring for a sick loved one or a personal healthcare experience they endured. 9. DRUG SCREENING
ADOM requires all prospective students to undergo a drug screening as part of the enrollment process. This policy helps ensure that our students adhere to the professional and ethical standards necessary for success in the field of diagnostic medical ultrasound and related healthcare services. Incoming students are required to pass a drug screening test and provide proof of a negative result with their admissions documents. Applicants should email drug screening results to adom@adomacademy.com OR schedule drug screening at ADOM Academy by emailing adom@adomacademy.com.
10. BACKGROUND CHECK
As part of the admission process, ADOM requires all applicants to undergo a comprehensive background check. A background disclosure and authorization form will be provided to students during the application process. Applicants should email completed background checks to adoma@adomacademy.com or request background check form at adom@adomacademy.com.
